Recently I found this website, where they uncover Photoshop Disaster in magazines, video cover or movie poster. Suddenly there is a third hand out of nowhere. A leg is missing or an actor has 3 elbows. Like in this example, where is this hand on Jessica Simpson’s shoulder coming from?
On Photoshop Disaster you can find small or really obvious errors. 
I hope I will never be on this website. Or maybe I should? That would mean my work is pretty popular and someone checked out my work.

WOW! Nice posting Alex, I hope my work never appears on that website though. I don’t think they have an approval process like we do here. You always need to let someone else proof your work, since you as a designer staring at the screen constantly, can easily miss details.

Thanks Mary. You are right, if you have someone who checks your work, it probably wouldn’t happen. At least not so often.
